The US tourism checklist calls for a completed and signed Schengen application form, a passport or travel document, and copies of every passport page, including previous visas and entry and exit stamps. Your travel document must be valid for at least three months after leaving Schengen, issued within the last ten years, and have at least two empty visa pages.

Provide an original and one copy of each document, in Dutch, English, French or Spanish, or translated into one of those languages.
